Fill and sign
Open the saved file in Preview on Mac, Adobe Acrobat Reader, or another PDF reader. Complete the applicable fields and add the required signatures.

Save and email it to Tim
Save the completed file with a recognizable name, such as LastName-DogName-Service-Agreement.pdf. Then attach that saved PDF to your email.
